Take into account opening a public pharmacy and opening a drug warehouse conditions
Notes
If license application was filed in governorates, the ministry will address health directorates to instruct inspection committees to perform the first and second inspection and transfer the inspection reports to the Directorate of licensing Occupational and Health institutions to complete licensing procedures.

Procedures
First stage
1- Referring to licensing Department of the Pharmaceutical Institutions and fill out an application form for moving pharmaceutical institution with the required documents.
2- Auditing documents and approving the application.
3- Documenting the application in the register
4- Transferring the application to the Director of health Occupational Institutions Licensing Department to transfer it to pharmaceutical Institutions Licensing Department.
5- listing the license requests on the inspections’ program.
6-The competent committee shall first inspect the site to make sure it meets the conditions of area and distance.
7- In the event of disapproval, the applicant is given a letter of disapproval reasons.
8- If conditions are met, a letter of initial approval will be given for the site.
The second stage
1- Submitting the second inspection request and attach the required documents.
2- Auditing the documents, approving the application and providing the license applicant with the necessary equipment and technical instructions.
3- Documenting the application in the register.
4-Transferring the application to the Director of Health Occupational Institutions Licensing Department to transfer it to pharmaceutical Institutions Licensing Department.
5- The second inspection committee shall inspect the pharmacy to make sure that technical conditions are met
Required Documents
For the first stage
1- Valid lease contract in the name of the pharmacist or title deed of the real estate where pharmacy is intended to be moved to.
2- Area plan for the pharmacy issued by licensed surveyor attached with a report about the pharmacy’s area and the distance from the nearest licensed pharmacy.
3- Approval of Municipality in which borders the pharmacy is intended to be moved to.
4-Valid site plan (date of issue within one year).
The second phase
1- Copy of the commercial registration certificate and trade name.
2- Copy of the pharmacy's seal stating (the trade name and the new address in detail).
